- Cut two 4 3/8"x6 5/8" pieces of blue striped paper (stripes running vertically) and ink the edges. Attach three red brads in the top right corner of one sheet. Open the accordion book to the third page and glue the papers to the left and right sides (placing the sheet with the brads on the right). Cut two short strips of light blue paper and ink the edges. Add a yellow brad to each end of the strips, then glue them overlapping the fold in the center of the page.
- Cut a 4" circle out of yellow cardstock, sand the edges and glue it below the brads on the right half of the page. Ink the edges of a 3"x4 3/4" photo. Cut two short strips of red stitched ribbon, fold them in half and attach the loose ends behind the bottom right corner of the photo. Glue the photo overlapping the yellow circle.
- Punch out monograms to spell the subject's name and ink the edges. Accent the first letter with navy stitched ribbon, then glue the letters across the bottom of the left half of the page.
- Write on a red label sticker and adhere it above the name.
