I start wider, then as I cut, I settle into a width that seems best for me and the item, usually 1/16", then go back and trim the wider areas when I'm done. I can always cut away more if it's too wide in certain areas. Also, I've found that 1/16" is about the widest I can do and still keep the mat even; any wider and I start getting wobbly with the scissors, cutting it less evenly.
The other thought I keep in my head is that it's a handmade card and it should look like I made it, not like a machine cut it. "Organic" and "custom-look" and other thoughts along that line also come to mind. :o)
